Signature Affidavit Doc Question

When filling out this document's Notary Acknowledgement or Jurat - a lot of times the wording says .... and that ........is the same. I have always put "he or she" after "that". I would like to know if there are other ways of filling this out, or is this the right way. Thanks!!

You would place both names in this area stating that the name that appears on the docs that the signers signed, and the alias are one of the same.

Technically as this comes before the notary certificate the borrower should fill that out. It is part of the document, not the notary certificate.

Eliz_CA & Jim_NH are both correct.

Lately, I have seen a Same Name Affidavit combined with a Signature Affidavit. It's something like:

My name is John J. Doe. This is my legal signature.
________.

I am also known as Jesse Doe. This is my signature as Jesse Doe.
_______.

I am also known as _________. This is my signature as ______
_______.

John Doe, Jesse Doe, and ______ are one and the same.

.
