Names on documents

Quick question...I know I've learned this already but I get confused with this. If the name on the document says John M. Doe, their driver's license says John Mark Doe, I know I'm supposed to have them sign as John M. Doe on the document but how do I write it in my journal, how it is on the driver's license? because that's how I've been doing it...
