Quick Question Please

I just completed my first signing and I am stuck on one part. I'm not sure how to fill in the blanks below. Any help is greatly appreciated!

State of KY __________________ County ss:

The foregoing instrument was acknowledged before me this __________________
by______________________________

_____________________________________________________________
known to me [or satisfactorily proven] to be the person[s] whose name[s] is/are subscirbed to the within instrument and acknowledged that he/she [they] executed the same for the purposes therein contained.

Here;

The county you notarized the document in goes after the state.

THIS 27th Day of April 2005 by John Doe known to me blah, blah, blah

Very standard notarization, IMO.

A,

Following up on the other post, if the signers are John Doe and Jane Doe, it probably would be wise or advisable to write out their entire names. In other words, don't take the short cut of putting down: John and Jane Doe.

A number of companies specifically stress this.
