Well I have tried to file complaints with the attorney general's office, grievance committee and the NY Department of State. I finally got a letter from the grievance committte and they suggested me contacting the DA's office for Nassau County (516-571-3800). I explained to them the situation and they are sending me a complaint form. I mentioned there are other notaries out there and that they could call to have complaint forms mailed to them if they are out of state. She did take his name and address from me as well. I also received paper work to file a small claims action against him with the court in his county.
