Countrywide docs

Has anyone had this problem. The wife is on the original Deed. For the refi they send all docs that Mr. & Mrs. are supposed to sign. However, wife does not have her name preprinted on the Deed or even written that she was on the original Deed. Found this out from husband. No instructions on how to sign her name. After a week the SS got in touch with me and asked me if I had a problem with this signing. Now they were missing pages. Went back with no instructions on how the wife was to sign and frankly after all the signings I had done I could not recall how she had originally sign the docs. All hell breaks loose when these docs are received. LO called. SS called. I got upset. Borrower was upset. Guess what? Got another call from another SS and with great misgivings went out again to these people. LO spoke to me like I was a slow witted individual. I noted that Mrs. was still noticeably absent on the Deed and any of the docs she was to sign. Called SS. SS called lender. I got a call back from LO and his assistant and was told this was a customary Countrywide procedure. I did a loan closing for Countrywide two days prior and wife had her name preprinted on docs. Anyone have this happen to them? Advise please.
