I have a co worker that would like me to notarize a will from her father. My first question is, does he need to fill out one of those template forms, or can he just free hand it? My second question is, do I use a jurat for that?

You don't say what state you are in. I believe CA is specific that a notary refers wills to an attorney.


I would not notarize a will unless it was prepared by an attorney, too many problems!

Also, no matter what document you are notarizing you can not decide which certificate to use, the signer had to instruct you as to whether a jurat or acknowledgment is required.

I'd call my SOS before notarizing a will.
