Please study your handbook. What you're asking is described in it.

CODE OF CIVIL PROCEDURE
§ 2094. Oath to witness; form
(a) An oath, affirmation, or declaration in an action or a proceeding, may be administered
by obtaining an affirmative response to one of the following questions:
(1) “Do you solemnly state that the evidence you shall give in this issue (or matter) shall
be the truth, the whole truth, and nothing but the truth, so help you God?”
(2) “Do you solemnly state, under penalty of perjury, that the evidence that you shall give
in this issue (or matter) shall be the truth, the whole truth, and nothing but the truth?”

Chris read 8206 (a) page 7, it states that you are to keep a record of all official acts performed as a Notary public. I would ID them as I would any other act. Good luck.

Thanks guys. I just wanted someone to confirm what I thought was correct. When you don't do something like this, it is nice to get a confirmation from someone to see if you are going down the correct path.
