Does anybody know where I can get a blank identification affidavit?

I got tired of all the different ID affidavits, some with no signature line but all the notorial wording.

I composed my own and carry copies with me.

I've run into a number of signings where there is no ID verification request. Since I have to comply with the Patriot Act, I verify ID and use my form.

Go to my website and click on "Forms". There you will find the standard Patriot's Act form used by a lot of lenders and my Visual Verification of Identification form that I use pretty extensively.
