Is anyone familiar with these forms?

The first is the Government of the District of Columbia office of Tax and Revenue and the other is Real Property Recordation and Transfer Tax Form from the District of Columbia office of Tax and Revenue. I have a signing this morning and both of these forms are in the loan package. These 2 documents need notarized (Jurat). There are about 100 blank spaces which the borrower needs to fill out like...their lot legal description, Mixed-use tax class, Homestead deduction, tax rate, property tax rate for abutting lots, etc. My experience with having these type of docs in the loan pkg is the borrower doesn't know half the info...thus leaving blank spaces...thus me not being able to notarize.

Any help would be appreciated.
