Limited power of attorney

Don't yell, but I have a limited power of attorney that says this instrument was acknowledge before me on (date) by, (blank), notary public. In the blank do I put my name or borrowers? Then at the bottom it has where I sign and seal.

would need more of the wording. The word "by" would imply the name of the borrowers, but the words notary public clouds that. Give a few more words after public and we might be able to help.

blank, notary public! You would put your name as it states, notary public.

just change the form to an acceptable one for PA. that one isn't even close. Hand-doctor it or attach a separate one. I had a special ack stamp made that I use when I've got to change one.

as always, notarial questions are state specific and what is acceptable in another state is not always acceptable in ours, especially ours.
