Survey-I need data...please respond

It's that time again where I need to evaluate my business to start planning for the coming year. The data I need are:

1. How long have you been in business as NSA, in months?
2. How many (SS-TC-Lender) have you work with?
3. What is your primary mean of advertisement?
a. Notary listing service
b. Direct sign-up (with companies)
c. Direct marketing
