They have no place for me to sign and it only has a date and this:
Notary Public in and for _________________
I put "the state of Florida"
Do I need to fill out a loose Jurat and attach it?
Or is this one of those that you don't fill in?
I would be greatful for any help.

Beverly... If you want, fax it to me (813-779-2865) and I'll take a look. If it's the document that I'm thinking of, there's something missing.

Yep, it's what I thought ...

It is an incomplete notarial certificate. But it does have the proper certificate body so you can attach a loose jurat to it.

The funny thing with that doc is at the top it states, "use a separate form for each party", but there are two signature lines marked "borrower" and one marked "co-borrower". Must have been a Philadelphia lawyer who drew it up! Smiley
