Not sure if I should attach CA Ack or not.

" On December 13, 2005, before me, the undersigned Notary Public, personally appreared "name of borrowers" personally known to me and/or produced _________ as identification".

The rest is standard notarization space. I have never seen a Correction Agreement LPOA with this blank space for ID. Should I fill in "Drivers License" and then attach a CA Ack with notarization?

Not sure about CA, but here we would write in the type of identification used.

Yeah, I'm in CA so ...

it is not standard CA Ack wording so I don't believe I can notarize this page. I know the blank has to be filled in with "drivers license" (I am assuming ) but my question was more of do I attach and ack or can I write in the missing CA Ack verbage?

If the document is to be filed in another state, you can use the wording on the form. Refer to Civil Code 1189, Notary Public Handbook, SOS
