Authorization to change Notary certificate

A title company I have not worked for before enclosed a form with the escrow papers which would authorize them to "correct" my notary certificate. In the past I have been asked to sign forms that had all my notary info (name, commission number, date commission expires, etc.). I don't mind signing that one, but I just don't feel comfortable signing something authorizing a company to "correct" my certificate.

Has anyone seen this form before? Did you sign it?

Have never seen one; don't think I'd sign it. It's one of those things that make you wonder--like some forms I've seen where they want the notary to notarize their own signature???

I personally do not remember ever seeing one of these "forms". However, if I did, I would not sign it because Florida statutes forbids "correcting" a notary certificate, even by the notary. A new certificate (and a new notarial act) would need to be done in order to rectify an error.
