Companies asking for All Purpose Jurat with documents

I have a document signing tomorrow and they have asked for an all purpose jurat to be filled out, I guess so they have it if there is a mistake or something getd forgotten. Has any one had this experience and how do you handle it without loosing the signing.

You tell them that it's illegal for you to do that and if you lose the signing, so be it. Those aren't the types of companies you want to be working for anyway.

Toss it. I've never mentioned it and have never sent one back, blank or filled in. It has never been questioned.
