borrower unsure of numerous "names used"

Yesterday I did a signing. On the page that verifies all names you have ever used, the husband only had his one name. The wife had an identity theft issue and under her one and only name she has ever used, there were 6 other names used. She just wrote "Have never used this name before".....was that ok to let her write that?

As she is swearing to the information (this had a jurat - right?), then she can only sign what she can swear is true to the best of her knowledge, and if she has never used those names she is right to put that next to them.
