unsure about adding acknowledgment help please!!

if someone comes to with a doc. that already has a place for notary seal, do i still need to fill out a al purpose acknowledgment form??? help

No, unless there is a problem with the acknowledgement that has been placed on the docs in the first place.

I have seen these quite often when someone has come for regular notarizations. They have a form they have had to sign, and there is a place for a notary seal, but no certificate.
As we cannot determine what certificate to use I let the signer know the two different certificates and let them decide if they want a jurat or an acknowledgment.
