Affiliated Business Arrangement Disclosure
I searched and did not find any postings referring to this question. I have a closing on Tuesday. One of the documents in my package is titled "Affiliated Business Arrangement Disclosure For Title Insurance, Settlement/Closing, and/or Appraisal Services Provided Through An Affiliated Entity of Your National City Lender". It has spaces for the borrowers to sign and is dated with today's (Monday) date. The other documents are dated with the day of the closing (Tuesday). Have any of you seen this document before, what is it, and should I cross through the date and enter the date of the signing?

Just leave the date, it isn't a notarized document and if the borrowers want to change the date to the actual date signed it is up to them.
This document only discloses to the borrower what other companies they have a business arrangement with. Doesn't always mean that those particular companies are involved in the transaction. I have had them where in the disclosure it will have Fidelity as a title company they do business with, yet the title company would be something different.
