Loose Certificate Needed: Jurat with Affiant Statement

Does anyone have a Jurat with the Affiant Statement wording?

Can I make this on my computer?

Can I download it from anywhere online?

Anyone have one they can send as an attachment via email?

Need before 3 pm, just in case the signer does not have the proper notarial wording, etc.
