Name Affidavit

I had a signing tonight and the lender included a name on the Name Affidavit that wasn't his, couldn't get ahold of anyone at the title/ss office.
He refused to sign it since it wasn't his so I left the signature off. He did sign the part that his correct name.'

ex:

he signed is at Charley Z Doe but refused to sign next to Charlie Z Doe because he has never gone by that name and isn't his legal name.

Did I handle this correctly?

I have never personally had this happen to me, but frequently I'll see instructions from the SS indicating that if the borrower has never gone by that name, that they should write "never went by" where the signature should be.

Did he leave the signature line blank, or did he write something in it (such as n/a)? Before sending the documents back, I would contact the SS in the morning to verify how they want you to handle this. They may want you to leave as is, or go back and have borrower write (never went by).

I have never had this happen before so I felt really silly having to call! I am going to go back in the early am and have him indicate that he never went by!

I've had this happen several times. I've advised the borrower to write on the line "I have never been known by this name."

I've never had a problem.
