Sorry - hit the enter by mistake. Anyway this doc has 3 parts. The 1st part says: Executed on this day of
Trustee_____________________
State of Missouri City/County ss:
On this day of , , personally appeared to be the person(s) described in and who executed the foregoing instrument, and acknowledged that they/he/she executed the same as their/his/her free act and deed.
IN TESTIMONY WHEREOF, I have herunto set my hand and affixed my official seal in the county and State aforesaid, the day and year first above written.

My Term Expires: Notary Public___________________________

I think I fill in all of the blanks except the Executed---------Trustee part.

Now the next part starts exactly the same except after personally appeared it has (name of officer), (title of officer), (name of corporation) known to me to be the person who executed the within in behalf of said corporation and acknowledged to me that he/she executed the same for the purpose therein stated.

My Term Expires: Notary Public_______________________________

Then the last part is just like the 2nd part except instead of Trustee, it has Lender.


Do I fill in any of the 2nd or 3rd part or only fill in the 1st part? Please help -- I have to UPS this on Saturday before 2pm. Thanks
