Survey Affidavit help

Can anyone help me on what should be filled in for a Survey Affidavit?
"There have been no new improvements made to the referenced property since the survey of said property dated_____________ which was made and issued by___________."

I am assuming this would be filled in if he had a survey? And if he didn't have a survey, would he put N/A in the lines?

I usually have them get out their last survey and have them fill out the information from that. If they can't find it, they usually write "unknown" or something. I leave it up to them how they fill it out if they haven't got their last survey. They are the ones swearing to the truthfulness of it.
