signature statement

I did a signing for Downey savings and loan. It had a "Signature Statement" page which reads:

I, John Doe
hereby certify that my name as indicated below, and said signature is my legal name.

This is all the page says. Does it have to be notarized? If so would i attach an acknowledgment?

No, a few laon docs have similar signature pages like that, that don't need notarizing.
