strange notary form question

Could some one help me out? I need to know how to fill in the blanks. This instrument was acknowledged before me on june 23,2005, by John Doe.(in the rare case of a Corporation or trustee it will continue "in my capacity of ----------for-----------.")

You'll see this again. The ()s cover when the person signing as a trustee or corporate office. Example. A family trust name Sam Doe trust number 1. would fill in "in my capacity as trustee for the Sam Doe number 1 trust".

For a corp, would be . "capacity of president of John Doe corporation."

So for a plain john doe do you leave them blank or maybe line throuhg them?

My rule of thumb, "When in Doubt, Leave it Out." If you have your own states acceptable wording on a loose certificate, use it, cross out the preprinted wording. As you need to complete the certificate as per your own states regs.
