Okay at the risk of being flamed.

I know this is a silly question but how would you fill in the blanks?

On the 26 day of May, in the year 2005, before me personally came ___________ to me known to be the individual described in and who executed the foregoing instrument, and acknowledgement that_________________ executed the same.

I'm sure I will have lots of folks telling me I don't know what I'm doing. I know this! That is why I'm asking the question.
