Signature and Name Certification

Here's a new one for me but I'm sure those of you who have dealt with Countrywide Home Loans can answer this. I received a HELOC doc pkg for a signing tomorrow that contains a Signature and Name Certification. This document is set up not as an affidavit but only for the borrower to certify to their legal signature. There is also an area below the borrower's signature where he/she is to certify that he/she is also known as:
While this looks like an AKA Affidavit, there is nothing on it for the notary. Any help would be greatly appreciated. BTW - I tried to contact the Lender but so far haven't been able to get in touch with her.

I think I just answered my own question - I found it on Msg#35674. I did try to look this up prior to posting but didn't put in the proper search topic.

I've had these as well. Gleaned from several similar posts - make sure they fill out the form themselves. Notaries can't help with the blank forms - kind of a bummer when your dealing with the elderly. I use that time to double check my MoJo.

I used to do those all the time.. dont notarize it, not necessary, they changed the format of sig cert so no notary is needed. Smiley
