Non-Foreign Certification - Transferee and Transferor

Anyone familiar with this 3-page form? I have a first and second and this was included in "miscelleanous docs" e-mail.

"This form is provided so that the buyer and/or seller in this transaction can certify compliance with the Foreign Investment in Real Estate Property Tax Act (FIRPTA) to the escrow agent and/or buyer. Transferee/Buyer must retain a copy of this document untiol after the fifth taxable year following the transfer."

There are the following sections:
1. Transaction on person residence not exceeding $300,000.00
2. Certification of Non-Foreign status by individual (I am not a non-resident alien for the purposes of U.S. Income Taxation)
3. Certification of non-foreign status by entity
4. Certification by buyer

There isn't any place to notarie, but there is a heck of a lot of blank lines for Borrower?? to fill in.
