Notarizing letter for minor leaving the country

I got a call from an individual that needs a letter notarized letting their minor son to travel out of state with another family. I live in Pennsylvania is there a form or should the parents of minor type a letter?

check with your SOS office. I did and found the only thing required of me was acknowledging the signature on the "permission slip."

In NY it is the same as Pa. Just have to notarize the permission slip.

As a notary is isn't your place to tell these people what they need to do in this situation. All we can do is notarize what they bring us, not tell them what it is that they need to bring us.

Thanks everyone. Eventually I ended up looking in PAN yesterday and found a form and the customers perferred this form instead of what they had written. I did not suggest anything I just showed the form as a sample.
