Those who have done CitiBank
I have a "Notice Of Immediate Closure Of All Lines Of Credit" form as the first page of my loan documents. Is the borrower just to sign and date this or are they each to fill in the "I,__-, hereby acknowledge that I am paying off..." part of the page?

Just have borrower sign and don't worry about the blanks. Title will fill in the blanks, attach the original payoff with the check and send it off.
That being said, IF the TC requires this closeout letter to be Notarized, you would then have the borrower do one of three things: line through the blanks, fill in the blanks using info. found on the commitment or call the TC and request the info. to fill in the blanks.

there is no space for notarization, another question...
same citobank package. page 6 of DOT. "Request for Notice of Default and Foreclosure Under Superior Deed Of Trust or Mortgage. Thisis on the top of the page and then the second part of same form is a "Request for Reconveyance". No printed names just a Date:_______ ____________. Do they sign this?
