General affidavit Need Advice Quick!
I have one of these in a pkg that does not have any notarial wording, it is a form that states the borrower has not incurred any new debt since the 1003. I just had them sign, however now I'm second guessing myself. There was no one to contact at this hour. TIA

Perhaps it doesn't need to be notarized. If you aren't comfortable sending it back without an acknowledgement, attach a loose one. The lender can throw it away if they don't need it. (I'm assuming I understood your question correctly)

Bad advice...
>>> If you aren't comfortable sending it back without an acknowledgement, attach a loose one. <<<
NEVER, NEVER attach a loose certificate unless you are replacing an existing one or are directed to do so by the document custodian. Simply attaching a certificate because "you think one is needed" is UPL.
Quite often, there are affidavits that do not require the signatures to be notarized. Do not second guess the lender or title company assuming that an acknowledgment or jurat is 'needed'.

The documents in the package are not yours to decide if you notarize them or not. If there is no notarial verbage provided by the originator of the docs, then they don't want them notarized. Keep it simple for yourself!
