Signature/Name Affidavit- WV

I did a search and cannot find my specific question answered. I know this is silly, but please confirm for me. Does the borrower sign each of the a/k/a names as they are written? I recently had a borrower refuse to sign the line with her former married name. She refused to sign the name since she is remarried and went through a nasty divorce. She would only sign the line with her current name. Thanks for your help.

The answer is yes and no !!!!!

Some lenders/TC require each AKA to be signed exactly as the AKA is typed.
Others require that the borrower acknowledge that the AKAs are correct (have been used in the past) but no signature required.
Some lenders/TCs require the document to be notarized.

I'd put a note attached to Affidavit explaining why one AKA was not signed. You did you job.
