? on 2nd Mortgage Deed of Trust

In the middle of the page of page 6 of the Deed of Trust it reads "Request for Notice of Default And Foreclosure Under Superior Mortgages or Deed of Trust" then at the bottom it has a signature line. Who signs this?

That's only for default

That is a corporate acknowledgement in the event of default. The borrowers will sign past that point and you will notarize the acknowledgement which follows the borrowers' signature line. That default ACK is very confusing because it falls in the middle of the DOT
