Just as a note, often the borrower will hand me the check or deposit slip to get the info - routing number and account number. I do not write the numbers in. I hand the borrower the form and have them fill it in. I'm not about to expose myself to being at fault if, because I wrote the number incorrectly, the money doesn't get transfer properly.

I do the same, plus I staple (not paper clip) the voided check or deposit slip to the form at the table.

When having them put thier information on the form besure that they are taking the ABA from a check and not the deposit slip. For wire transfers you need the ABA that is on the check the one on the deposit slip will not work (they are different)
