All Purpose Acknowledgement

I have a signing for today and there is an all purpose acknowledgement at the bottom it says Attention Notary: Although the information requested below is optional, it could prevent fraudulent attachment of this certificate to another document. Then it has Title of Document Type: , Number of Pages: Date of Document : , Signer(s) other than name above:

What do put in these blanks?

If it a mortgage that you are using this for then you would put the word "mortgage" in the type of doc and the number of pages where it asks for the number ofpages. As far as othersigners, usually you will not have this filled in. Be sure and put the names of the borrowers at the top and check which kind of identification you are using. Also, don't forget to put your notary seal where it is requested. Hope this helps.

If this is a loose certificate, then you fill in the information with what doc you are notarizing. Example if it is an aka affidavit, then in the title or type of document you would enter aka affidavit. For document date, you would put the date the notarization took place. This is just information, so that if the loose document gets separated, they know what the acknowledgement belongs to.

I don't do all purpose acknowledgements. It is strictly prohibited in Florida to give anyone a blank acknowledgement that is not attached to a document.

Amending my own post, Monica's advice is correct if you need to use the all purpose acknowledgement for a document in the package. If you aren't using the acknowledgement for a document in the package, then it does not get completed. Some notaries keep them and use them later if they need an acknowledgement and other notaries (me included) cross through them and send them back in the package. It's your choice.
