I've heard of it. From what I've read, it is only used in states which recognize domestic partnerships. I'm sure each state would have their own form since definition would vary from state to state.

Sounds like 2 people want to establish a domestic partnership and need a state acceptable form notarized before the partnership will be recognized and the benefits take effect. Form probably has to be filed with the state.

Here is the link to the California Secretary of State site regarding registered domestic partnerships in CA. You will note that the Declaration needs to be completed, then notarized and mailed to the SOS. Hope this helps!

I have seen this on every Countrywide set of loan docs I have done recently.

because to sum up the wording of the document, it is for same sex couples or one or both are over the age of 62 and meet eligibility under the social security act for old-age insurance benefits. I wonder why every Countrywide loan would require notarization of this form if it doesn't apply?

The document being referred to in the Countrywide docs is not the declaration itself. It is a form asking the borrowers if they are in a registered domestic partnership or not. It is not a notarized doc. I believe your original question is regarding the actual Declaration that someone is preparing, as explained on the SOS site already noted. Hope this makes sense.
