said State of Florida and signing is in New York. Can I cross Florida and write New York or I need to attach a loose form?

If you are the notary, yes, you can change the venue from FL to NY without needing a loose form. Simply cross through FL, write in NY and initial the change.

You can change the venue to New York without any problem. However, you must ensure that the wording of the notary certificate conforms to NY statutes in form, fit and function. I do believe NY has pretty specific wording for an acknowledgment, especially on the mortgage or DOT.
