Help...Customer Identification Verification Form
I have a signing this evening and one of the forms includes a "Customer Indentification Verification" Form. There is a section for "Certification" their 2 forms of ID have been examined and appear geniune. However, there is no notary language and there is only a spot for signature, name and date. Has anyone had experience with this document? Does it have to be notarized or just "witnessed"?

I am still new at this...but I have been told to fill them out. Not to notarize them. And sign them myself.

Thats right. Just fill them out. that form is becoming very common, at least in cali. No notarizing, just checking to make sure you seen ID

I fill them out and sign them adding Notary Public after my name.

These are common in the packages I do (in Calif). I fill them out and sign them. Just make sure that you're getting the ID they ask for. One of the lenders I work with wants a second form of ID (utility bill, mortgage or bank statement etc.). If they require a second form, it should say somewhere at the top of the sheet.
Also, I don't sign as "Notary Public", I sign as "Signing Agent". I don't want it to look like I'm signing in the capacity of a Notary on this form since it isn't notarized.

excellent point. As I am not signing the form as a notorial act, I don't want my capacity as a notary involved. I also sign that one as "Signing Agent."

Yes, I also do sign as a Signing Agent since the form is not notarized. This form is very common for new accounts in CA.

I have been seeing this more and more. Instructions from one company is as follows
To help the government fight the funding of terrorism and money laudering activities, federal law requires all financial institutions to optain, verify and record information that identifies each person who opens an account.
The notary will need to provide his/her signature indicating taht they have verified and completed the form.
Notary seal is not required
