Hypo Medical Forms?

Has anyone ever notarized these type of forms?

I got a call to notarize a set and I am not sure what they are?

This is just a guess....

I have 2 thoughts -- #1. It might be a consent form. #2. It might have to do
with the required shots (since the outbreak of the mumps and such).
However, this is just a guess.

If it is HIPPA, ...

... it will not present any difficulties. Just the usual notarial block.
