I need to notarize Affidavit - Death Of Joint Tenant, borrower signature line show like this: ______________________________ John David Doe, surviving Joint Tenant . Mr. Doe need sign is full name and then write surviving Joint Tenant?

I would actually call the title company on this one. I've never seen anyone sign a deed this way, but I'm in Florida. CA may do things differently.

It's not the deed, but an affidavit. The property was probably held (guessing on my part) as joint tenancy with rights of survivorship, which is often done when the property is not held as spouses. From those that I've done (I think only two or three) in this situation, the signer simply signed his name, as the affidavit is what explained the relationship (joint tenants), not his signature.

Agree with PAW
that is the benefit of holding title as Joint Tenants...the remaining JT hold title in full upon the death of the other joint tenants. No need to add a title after his name, the name is sufficient.
