DOT Initials etc.

Sally Sue Smith is signing tomorrow, the Note shows her as "Sally Sue Smith", the DOT shows her as "Sue Smith K/N/A Sally Sue Smith". My question is...how does she initial each page of the DOT? Incidentally, her signature line shows the K/N/A. TIA
