Notary does not = provider of legal forms!
Why do notaries keep thinking it's their responsibility to supply forms for their customers' every need? They provide it, you notarize their signature on it. Period. Otherwise you can get in SO much trouble if you provide something and they trust you that it's the right wording but it's not. Not to mention that there may be other related issues that someone with training in immigration or whatever would know to ask about. If someone needs a form and you really feel like being helpful, refer them to google and say, "I'm sure you can find a form online somewhere."

Sharon's right
Unless your state law is very different from my state's law, notaries public must not provide legal advice. As much as we all want to help people, and there are times when it is very tempting, we cannot exceed our limits. Sooner or later having provided a form will be taken as having given legal advice, especially if something goes awry.
Remember, we can't even tell the signer whether to use a jurat or an ack!
