Wording question for VA Notaries

I've done a search and didn't find this one.

Acknowledgement:
This instrument was acknowledged before me this ___ day of ________ by _____________.

My question is does the BO's name go in that last blank? When I read it with out the date it seems that it should read with the BO's names but I just want to make sure. Thanks.

Yes, that's what it looks like to me. The borrower's name goes in the last blank.
