Ack or Jurat?

I have a Application for Vital Record that needs to be notorized. On the top, states that the signer "Swears under penalty of perjury", but on the bottom is acknowledgment wording. Do I use a Jurat or Acknowledgment form for this?

When there's ack wording use an ack. Don't let the fact that the doc says swears on it throw you off. I know when I was new I over thought those types of docs. What's in the body of the doc doesn't matter. If the wording is ack perform an ack.

No problem. One of the most important lessons a seasoned pro taught me is this. "Ours is not to ask why but to get the ink dry". Don't over-analyze your docs. Keep it simple. And yes when one is new to the field it is nice to just think it out loud and get a yes even when we know the answer. Good doc question, that is what we are here for.

Sort of similar thing here: I was told by a title company that my ack form is outdated! I got them in 2004. Any ideas? Thanks. Lauren

Lauren, have them fax you a copy of what they are talking about. Then look in your state handbook and see before proceeding. Also if you could put your state abreviation in your handle you will recieve more state specific answers.
