Has anyone had this request?
Just got a call from a guy that needs to take a test online for traffic survival school. The notary needs to be in attendance while he takes the test and notarize a doc saying I witnessed the fact. Has anyone heard of this?

THAT'S CRAZY MAYBE YOU COULD SIGN IT FIRST, THAT WOULD FREE UP ALOT OF TIME TO DO REAL NOTARY WORK

I've had something similar.
It was the husband of a friend and he had to take a test for this company he was trying to get hired on to. I had to be there to observe him taking this test and make sure he was not cheating. The test had the notary wording at the end.

So does the test taker just need to get the test notarized or does the notary actually have to watch the tester take the test? that would be brutal

Unfortunately its too true. Some courts require that the identity of the test taker is verified so they require that you take your traffic school and then you go to a notary, some traffic schools have agreements with agencies like Kinko's where they fax the test to Kiknko's and then you take the test at the shop. I've done a few in the home and it is boring but it can be lucrative. There is nothing like actually watching someone to take a test. I don't know how some people do proctoring.

Seems like you saying that you saw him take the test would be a "witness" function - not a notary function. You could certainly ID the guy and notarize his signature (on a doc provided by the traffic school) with an ack or a jurat, BUT, if I'm reading this correctly, the traffic school wants a statement from you like "I saw Joe take this test" and that's not a notarization (unless you swear to it and have ANOTHER notary execute a jurat), so you shouldn't sign it Yourname, Notary Public. I would ask the guy if the school provided him with a form and make sure they aren't asking you to do something that is outside the scope of a notary.
