Good question, Charming. If I did not know better, I'd think it was a test question. But, I am not certain about that. I cannot imagine why else the question would be asked. If it is in the package, it is obvious what the answer is.

I thank you for the search recommendation, but it doesn't appear to be working, and I have placed a call. This is not a test and no I haven't seen all the different lender froms. Can someone directly answer the question. Pleeeeease.

Now I agree with the sarcastic responses and if search wasn't down I wouldn't bother. It's obvious your new and haven't come across this till now. It's clear when looking at the doc that the borrower is to fill it out and sign it. If at all possible call the borrower and ask them if it's possible to fax it over to them. This doc can take from 5 to 30 minutes for the borrower to fill out. If you can't fax just give it to them to fill out while you are entering your information in your journal. They can finish up while your double checking the docs once they are signed. Of the packages I have done whenever there's a AOI many times there's also a form for them to fill out there home-owners insurance info. Give the borrower a heads up that they will need to be prepared with this info on hand for filling out docs.
You'll get used to the docs that are usually in packages with some experience. Until they pour over what you get, it's inevitable that some lo or tc will come up with something you haven't seen before.

This is a good question to ask . . .
. . . if you mean does the Signing Agent fill out the form or does the borrower. I ask the borrower what other names they go by and write them myself, as my printing is very neat and I can't count on the borrower's. The borrower, of course, signs where indicated.
You should write N/A on any lines where a name is not written.

Or...in my experience, "no aliases" on the first line works too. It may be a lender/TC preference. It is best to ask the hiring party what is expected. We can give good information, based on our experiences, but it all boils down to how the hiring party wants it done. JMO

"No aliases" would work if the borrower doesn't go by any other name(s), but I have found that N/A entered on unused lines has been acceptable for all companies for which I have done so.
