Consent form
I just got a call from a guy who wants to have a note authorizing his wife to take their son out of State notarized. He wanted me to help him put the consent form together for him, which I declined. I have never notarized this type of document before, can he hand write it and have me notarize his signature? Is there a specific form?

I don't know of a particular form for that, but there is a form for people to fill out and notarize when children need to get passports and only one parent is available. That form is on the national passport website. That might be a good starting place for him, and I believe it all ready has the notarial wording on it.

Ok. I did find another site, "www.Travel forms.com." He would have to fill it out on the computer, print it, and then present it to the Notary. Thanks for your help!

Donna,
Each state may be different. I'm in Alabama and I have notarized several of these documents. One time the parent had a lpersonally written and typed up by him. I watched him sign it and I notarized it after verifying ID. Another time a parent brought a handwritten note to me. Again I notarized it. But that's here in Alabama.

I did the same here in CA and it worked. I read there is no 'law' as to how it needs to be written up.
When the client wrote up his consent, I just put the jurat stamp under it and had him signed.
It did work.

Is there a specific form? No but there are many preprinted forms floating around.
