Form DS-3053 accompanies passport applications of children under 14 whose parents (or guardians) cannot both appear in person at the local passport office at the time of application.

Marlene, Thank you for posting this. It makes sense to me now. I called my SOS about it and they said to attach a loose certificate, but as the article said, I would be worried about it getting rejected.

You're right, when dealing with government forms, don't attach anything they don't ask to be attached. It can be an automatic rejection.
