I realize you are stressed over this, but try to think this through on your own.
RE: ACKNOWLEDGMENT
Did you place your seal/stamp on the acknowledgement of the deed of trust? Yes or no?
Should you ever just send out acknowledgments without people in front of you acknowledging they signed a document?
If you do not carefully think this through you will be likely to make this mistake again IF you made a mistake at all.

Seconding (as usual) Brenda
There are some firms which will send a blank, general-purpose ack along with the rest of the docs. They'll want you to fill it out "just in case", and not attach it to a specific document (i.e., not specify a document name or number of pages when you fill it out). Filling out such a form is A Bad Idea. You will have no control over it. An unscrupulous person could attach said ack to any doc at all.

You didn't say who "they" is, but be careful. You do need to complete a CIV in most cases, but once in a while they will want you to stamp it or "notarize" it. Keep in mind that we notarize signatures, not documents, so if the borrowers do not sign that form, no can do. (You can't notarize your own signature.) In CA, we can use our stamp and title ONLY for the purpose notarizing a document.

Get Proof!
Make them prove to you that you forgot to notarize something. Have them fax back to you the docs they claim you didn't notarize. Then, IF you made a mistake, fix it.
