What is printed on the document?
Whatever is PREPRINTED on the document. If there is nothing preprinted, then you must be told what is needed. NEVER, NEVER, NEVER attach what you THINK is needed, as that is considered UPL in most jurisdictions. The signer, author, or custodian of the documents must tell you want they want as far as the notarial act.
To assist the signer/author in determining the appropriate act, look for keywords in the document, such as "sworn" or "acknowledged". Then explain the purpose of each act, acknowledgment and jurat, to the signer/author and let them determine which to use.

Additionally, sometimes loan documents are titled "Affidavit of This or That" but do not contain ANY indication of requiring notarization.
Many times it's because the document was revised at some point, the notarial verbaige/requirement was removed by the doc originator - but the title of the doc was left 'as is'.
