quick question

Can a notary public notarize a sworn statement if the person refuses to take the oath "swear to God" because of religious reason?

use an affirmation

If a person does not want to perform an oath due to a religous reason you can normanlly use an affirmation. Something like:

"Do you affirm under the pains and penalties of perjury that the document entitled XYZ which you have signed before me is accurate and true?"

Check your state laws, but this above form is acceptable in New Jersey.

I use the following oath/affirmation: "Do you swear or affirm that you sign this document of your own free will, you understand what you are signing and to the best of your knowledge the statements contained in the within document are true and accurate?"
